$0 Preventive Services (Annual Physicals)-Covered at 100% (also includes weekly savings on health insurance).

Health Savings Account

An account funded by pre-tax dollars to help you save and /or pay for future medical expenses. The funds in the account roll-over from year to year and can be used as an additional option for savings for retirement.

Premier Initial Contribution: $25 Premier Annual Contribution: Employee Only $176 - Employee +One $507 – Family $705

Health Insurance Allowance

If you do not wish to elect health insurance coverage, the company provides an allowance (must provide proof of other coverage) Single-$15.00 / week X 4 weeks, Employee & 1 or more $25.00 / week X 4 weeks

Preventive Services paid 100% - $50.00 Deductible Individual/$150 Family

Vision Insurance

Coverage is funded 100% by Premier Tooling & Manufacturing, LLC.

Short Term Disability

Provided to all full-time employees, 60% of weekly pre-disability pay with a max of $1,000 per week.

Life Insurance

Provided to all full-time employees with a benefit of $25,000.00.

Additional voluntary insurance is also available at your own expense through payroll deduction.

Voluntary Accident/Critical Illness

Offered to all full-time employees at his or her own expense through payroll deduction.

401(k) Retirement Plan

Offered to full & part time employees 18 years of age or older following 90 calendar days of employment on an enter date of the 1st of Jan, April, July or Oct. Premier Tooling & Manufacturing, LLC will match 100% up to 3% plus an additional 1% if you choose to defer up to 6% of your wages.

Vacation

Provided for full time employees (a percentage is available for part time employees). The vacation schedule is as follows:

At time of hire (1st of the month following 60 days of employment) = 2 weeks (80 hours) paid vacation

5 years of full-time service = 3 weeks (120 hours) paid vacation

10 years of full-time service = 4 weeks (160 hours) paid vacation

10 Paid Holidays

All full/part time employees qualify upon completion of the evaluation period (1st of the month following 60 days of employment and paid according to your work schedule).

Bereavement

Provided for full time employees upon completion of the evaluation period (60 days) in the following matter:

Immediate Family = 3 days paid & 2 unpaid working days off.

In-Laws, grandparents, grandchildren, stepparents, stepbrothers & stepsisters = 2 days

Anyone other than the above-mentioned relations = 1 excused unpaid working day.

Referral Program

Receive a bonus in the amount of $1800 (pre-tax) once the referral completes 6 months of service.

Attendance Award

Once a month full & part time employee can qualify for additional dollars; all for planning absences one week in advanced and working their normal scheduled hours. The award is paid out every month for qualified employees.
